To register for a Vietnam Visa online, you need to provide these information:

REGULATIONS ON ENTRY/EXIT FROM VIETNAM
The law on entry and exit of Vietnamese citizens does not specifically mention cases of being banned from exiting the country. Currently, it only provides detailed regulations on cases of temporary exit from the country.
The fines for violating regulations will be determined on a case-by-case basis and will range from 500,000 VND (equivalent to $20) to 16,000,000 VND (equivalent to $641).
The duration of the violation will determine the amount of the fine as follows:
+ 500,000 VND for 1-3 days,
+ 1,250,000 VND for 4-10 days,
+ 4,000,000 VND for 10 days to 1 month,
+ 10,000,000 VND for 1-3 months,
+ 16,000,000 VND for violations over 3 months.
